Boston Militia Women's Football 2013 Players' Forms
All Players, Please Note
All forms and dues payments are to be submitted to the General Manager, Frank Ferrelli
prior to entering the field for practice or tryouts.
All forms required from new and returning players.
All forms required at initial tryout or practice that the player attends.
Player initial dues required payment of $100 is due on February 1 and the balance is to be submitted by March 1.
Players MUST provide proof of medical insurance by making a copy of their medical
insurance card (front and back) and submitting it to the General Manager prior to
participating in any try out or practice session. The
WFA Registration fee of $25.00 is required by March 1. WFA Registration is required in order to participate in games. If a player is not registered with the League, they may NOT participate in any WFA game. All checks are payable to the Boston Militia.
WFA Registration will cover players medical insurance during team football practices
and games only. This is a limited medical insurance coverage.
