Boston Militia Women's Football 2013 Players' Forms

All Players, Please Note

• All forms and dues payments are to be submitted to the General Manager, Frank Ferrelli prior to entering the field for practice or tryouts.
• All forms required from new and returning players.
• All forms required at initial tryout or practice that the player attends.
• Player initial dues required payment of $100 is due on February 1 and the balance is to be submitted by March 1.
• Players MUST provide proof of medical insurance by making a copy of their medical insurance card (front and back) and submitting it to the General Manager prior to participating in any try out or practice session. The
• WFA Registration fee of $25.00 is required by March 1. WFA Registration is required in order to participate in games. If a player is not registered with the League, they may NOT participate in any WFA game. All checks are payable to the Boston Militia.
• WFA Registration will cover players medical insurance during team football practices and games only. This is a limited medical insurance coverage.
